• A referral with a current Workers’ Compensation Certificate signed by a Medical Practitioner or Nurse Practitioner to cover services.

• A referral to provide allied health services under private health care cover

DVA

• A completed D904 form OR a referral with the following information: patient name, date, DVA File no. and card type (include accepted conditions for white card holders), relevant health history, condition to be treated, confirmation that you are this patient’s usual GP or a medical specialist, other treating providers and, your name and provider number.
